Failure has occurred once again in our operations department. Minor negligence at each step snowballs into an irrefutable disaster. The responsibility for the mishap is structural, attributable to every team member in the process, but I am taking most of the blame. If only the company system we’ve built had no chinks in the armor, I wouldn’t need to reevaluate the business.
