Bad form due to an over-emphasis on quantity and not quality. Good coaches should prevent this from happening, but it rarely happens.

Overhead exercise in high quantity with poor mobility. Also, all movements involving hanging - pull up, chest to bar, bar muscle ups, even toes to bars are a big impact.
