Hospital or urgent care visit...can't see everything, but from what I can see chief complaint was probably chest pain.

Check your local laws. In MN hospitals are required to send bills within 30 days, but states like FL give hospitals up to 5 years.

someone could have used your identity or insurance to get medical care. contact billing to get more information then contest it.

I got a $10000 bill one time from Texas valley…. For a baby delivery. I don’t have any kids. I kept calling and nothing was being done. Finally, I got tired and told them I would pay them, but they would have to give me a baby. The harrasment stopped.

Pro ably identity theft bc I went to file taxes one year here in Ohio and was surprised to find out I had taxes coming from an A-1 Dry Cleaners in Dayton. I don't live in Dayton, I'm just north of Columbus and never worked at a dry cleaners. I claimed it anyway per the HR Block lady and never had another issue of things like that coming up. Very odd
