It is absolutely not synonymous with a DNR. Literally everyone I know is a donor (in the UK you have to opt out, not in). Not a single one of them has been a DNR. It's a really bizarre concept that doctors won't try to help you if you're a donor. It just couldn't be further from the truth.

Yes, this is one of the lies people tell themselves to make excuses for being against opt-out. There truly is no legitimate reason against it, it’s all just versions of “but I don’t want to”
