For arguments sake lets say she does not give back the kidney and they still divorce, presumably amicably. Later on in life… the man gets a similar kidney issue. Is the ,then, ex-wife obligated to give back the kidney?

Not fair. I looked for information about this and she asked for divorce alleging he commited domestic violence and infidelity. So sad that are people in the comments defending this man despite he used the kidney donation as a way to trap and abuse her.
