It’s a histamine reaction. When the body makes scar tissue there’s a cell called the mast cell that’s more prominent in scar tissue. It can release histamine which makes it raised and itchy. A lot of things can trigger a reaction: heat, sickness, stress, allergies, bug bites, autoimmune diseases, etc.
