When I was covered under my employer’s insurance plan, I was the Benefits Director. We were self insured, as most large corporations are (which doesn’t mean what most people think it means). In any case, we were able to tailor coverage to a large extent. When I proposed that we cover colonoscopies at 100% after the deductible at any age, when recommended by a doctor, because who the heck would have a colonoscopy if they didn’t need one, our executives agreed we should cover it. So we did.
