I have. My instructions are precise and detailed. I have iterated the instructions numerous times. It straight up ignores the instructions at various points. It's even evaluated them and said they're sufficient... then immediately ignored them in the next instance of running it. When I ask, ChatGPT has acknowledged there's no reason it should be ignoring the instructions, yet it does. And when I ask it to review the instructions and go again, it gets them correct. So... yep
