I will read the chart and ask you all the questions anyway. First rule of medical school: never trust anyone. Not the patient, not what the other person wrote down. We double check everything. It's a safety concern.

I understand verifying what's in the chart, but when a hcw acts in a way that's completely ignorant (ignoring my previously communicated ADA accommodation, for example) & questions whether I *really* need it when the info is definitely in my chart, yeah, we're going to have a problem.
