This weekend I made what will probably be one of my best pharmacy interventions in my career. I was working my weekend job at the hospital and the internal med NP contacts me to tell me a patient has an INR of 12 and needs vitamin K, but doesn’t know how to dose it so is asking me the dose. Having worked up all my 50 patients already- I didn’t recall seeing an INR that high so I double checked the patients INR and it was subtherapeutic at 1.1 and the PT was 12. (1/2)

I had to remind the NP that we use INR to dose warfarin not PT. This patient needed a dose increase to prevent clotting not vitamin K! Please thank your pharmacist and their great attention to detail to things like this, because had I just answered the question and not verified the lab result (as pharmacists are trained to do), that could have been a deadly outcome for the patient. Rule 1 always repeat lab test! A too full of not full enough tube can make a big difference! And run it STAT!

As an FNP for 25 years, I believe RN’s should have at least 2+ years full time work experience before admission to an NP program. Anything less is a disservice to patients & NPs

I’ve seen double digit INRs. Makes me think pharmacists should be dosing all warfarin.

I had a resident order subQ heparin on a patient admitted for hemophilia. I showed my charge nurse for verification I was reading the order correctly. Granted, it’s likely it was part of an order set that the resident didn’t catch while ordering 50 other things, but it was still a very dangerous order. Thanks for your attention to detail! I promise the patient would thank you as well!
