I will read the chart and ask you all the questions anyway. First rule of medical school: never trust anyone. Not the patient, not what the other person wrote down. We double check everything. It's a safety concern.

Medical records are static. Your body is not. Unless your reason for consult is a chronic condition, I won't bother with your chart and will ask you directly. First hand information is better always. Asking you will take seconds. Reading the chart will take time away from my contact with patients. I won't trust my information gathering on someone else. IT IS A SAFETY CONCERN WHEN WE ASK AGAIN, EVEN IF WE'VE READ YOUR CHART. Stop telling us what our job is. Just answer the goddamn questions.

Absolutely told a nurse who brought my son motrin despite it being listed as an allergy due to his kidney disease . I asked her did you even read his chart well yeah but this is what we were told to give him I'm like no maam go back and figure something else out you're not giving him something that will shut his kidneys down

I understand your frustration. I would be too. However, it would be much faster if we just read the chart and make a plan based on it. I always check the chart before seeing patients but still ask them the same thing. The reasons why we have to ask those repetitive questions are 1. liability & defensive medicine- If we make plan based on what the note say and things go south, it will become pointing finger game among health providers. Therefore, our plan is based on what you tell us.

I've stopped filling out the whole form. They're gonna ask me what meds etc etc so y take the time

Best practice is to review medical history both via the chart AND with the patient. You would not believe how many people wait until they've been seeing their provider for years before dropping bombshells like "yeah, I had *major medical event* just before I started seeing you." Or "I had *major medical event* right before my last appointment here, I didn't think it mattered enough to mention" meanwhile they had quadruple bypass surgery or total hip reconstruction or a stroke.

You obviously don’t know how much incorrect information can be in a chart huh

They ask these questions for your safety. It’s the same as your pre-op instructions will say “do not eat after x pm the night before” but the anesthesiologist will still ask you if you’ve had anything to eat since then. Extra confirmation is never a bad thing, even if it can be a little annoying.

I feel your sentiment but maybe I can explain why - They also need to hear you say it - human errors are made every day, it’s possible a mistake was put into your chart for you rather than for someone else, they want to make sure it’s accurate and your understanding of your disease processes that ail you is the same list they are seeing. It’s a double check - but this is why i use active reading and listening and say the list of what my understanding is and ensure we are on the same page
